on

investigation of an aviation occurrence with Ka-32AO helicopter, registered LZ-MOZ, owned by Scorpion Air Ltd Air Operator, city of Sofia, occurred on 28.08.2006 in the area of the town of Gombe, Republic of Turkey.

On August 28th , 2006 at 14:00 hrs in the area of town of Gombe, District of Kash, Republic of Turkey, a helicopter Ka-32, reg. LZ-MOZ was taking water from the Chaibogazi artificial lake during a firefighting flight. After taking water the helicopter loosed power and fell into the artificial lake and sank. The crew left the helicopter and swam to the bank, where crew members received medical assistance.

Командирът на въздухоплавателно средство (КВС) P2002-JF , рег. № LZ–DBC, в 09 :25 h на 05.08.2005 г. започва засилване на самолета за излитане и изпълнение на тренировъчен полет по кръга на летателна площадка „Долна баня”. Началото на засилването започва от грундовата част на ПИК 27, на 36 m от източния праг на асфалтобетоновата писта за излитане и кацане. След повдигане на носовото колело и отделянето от пистата, самолетът от височина 2-3 m се отклонява наляво от курса за излитане и напуска пределите на ПИК. В курс 230° губи височина като опира на грунда с двете колела на основния колесник. След повторно отделяне от земята самолета се срива на ляво крило, след което се установява в курс 160º.

investigation of an aviation occurrence with MD-82 aircraft, registered LZ-LDC, operated by Bulgarian Air Charter, occurred on 28.05.2006 during international chartered flight BUC 7105 Varna – Dresden.

Aircraft MD-82 reg. LZ-LDC took-off for a charter flight Varna-Dresden at 12:04 UTC on 28.05.2006 with 16 passengers on board. After take-off and climbing to FL300, just before leaving Romanian airspace the crew ascertained cabin altitude increasing (depressurization). The oxygen masks in the passenger cabin dropped automatically. Emergency descending was made to FL 90, the aircraft returned and landed at Varna airport. There were no consequences for the passengers, crew and aircraft.

investigation of an aviation occurrence with An-2 aircraft, registered as LZ –1217, operated by air operator „BASK AIR” JSC, realized on 18.04.2006 during an operational flight for aero-chemical works.

An-2 aircraft registration No LZ-12-17, was fulfilling a flight for ACW from an airfield at Vetrino village. During a working pass for spraying with herbicides at 4 km Northwest from the airfield, the aircraft commander noticed an engine vibrations raise and after that he ceased the work and made a circle at safe altitude in order to check engine operational parameters. The commander determined that it was impossible to increase the engine speed above 1600 rpm. Taking into consideration the distance to the airfield and the obstacles in all directions (high voltage transmission lines), the commander made the decision for landing at the place, were he was. The crew made three approaches, selected a proper place from the air and landed.

The landing was without any consequences for the crew and the aircraft.

investigation of an aviation occurrence with Cesna 150L aircraft, reg. LZ-ASF, operated by AIR SCORPIO Ltd. air operator, realized on the 17.04.2006 during a training flight at Gorna Oryakhovitsa Airport.

CESSNA 150L aircraft, reg. LZ – ASF, took-off for the execution of a training circling flight from Gorna Oryakhovitsa Airport at 15:10 local time. During the execution of the last (5th) circling flight on reverse course just before the turn into base leg an engine flameout occurred. After an unsuccessful attempt for an in-flight engine start up the aircraft commander decided to land on a chosen from the air landing ground and informed the air traffic controller at ATC-Tower of Gorna Oryakhovitsa Airport. The landing on the chosen from the air landing ground, a field at the turn into base leg area, about two kilometers from Gorna Oryakhovitsa Airport runway, was without consequences for the commander and the aircraft.

investigation of an aviation occurrence with An-2 aircraft, registered LZ –1178, operated by air operator „Kapoli Air”, occurred on 11.04.2006 during a flight for aerial works in the Kozlovets village area, Municipality of Svishtov.

The aviation occurrence occurred during the twelfth flight of the day for aerial works on 11.04.2006 at 15:55.

The aircraft hit the ground during a turn and was destroyed, and a fire emerged. As result of the fire, the fore part of the aircraft was destroyed. The commander and a person onboard died. A child being in the cockpit was seriously injured and was hospitalized in a hospital in Pleven.

investigation of an aviation occurrence with Aviatica-890 SH aircraft, registered LZ-AVB, owned by MORENA-CHAR Ltd, City of Stara Zagora, occurred on 11.04.2006 during aero-chemical works (ACW).

On 11.04.2006 the pilot of single-seat aircraft AVIATICA 890SH, registered No LZ-AVB carried out a series of flights for ACW. After refilling the chemical tank with herbicides he took-off from an old agricultural landing field in the Pet Mogili village area, flew over the village heading Southwest for treatment of a field at about 500 m to the west from the village. The flight altitude was 15…20 m. Just at the southern end of the village at a distance of 40...50 m before the perpendicular high voltage electric line with 15 m high pylons, the aircraft made a sharp turn of 180° with steep descent and hit the ground with a right bank. As a result of the impact the aircraft airframe was destroyed, the pilot was injured and died before ambulance arrival.

 

 

 

 

 

 

Remark: During the past year another aviation accident occurred. It took place in Cape Verde and the investigation is still being conducted by Cape Verde's national aviation authorities. As to the moment only a Preliminary report has been sent to AAIU.
